GEMBA DECISION RULEPARETO LOSS DIAGNOSIS

Reported Performance is 114.1% (exceeds 100.0%). Do not cap values artificially. Perform engineering audit: (1) Re-verify if nameplate cycle time (12s) is overly conservative post-Kaizen, (2) Inspect optical/proximity counter sensors for double-pulse chatter.

* Recommendations derived from actual reported input losses; requires gemba verification by authorized plant personnel.

VERIFIED INDUSTRIAL FORMULA

Shopfloor Mathematical Foundations

Every calculation engine adheres strictly to established industrial literature, eliminating end-of-shift spreadsheet disputes.

ISO 22400-2 & Nakajima

OEE = Availability × Performance × Quality

Official lunch breaks and shift briefings are deducted from Planned Production Time. Machine halts reduce Availability, while running below nameplate speed reduces Performance.

Toyota Production System

Takt Time = Net Available Time ÷ Customer Demand

Takt Time is the heartbeat of the plant dictated by customer purchase frequency. Stations are engineered at 85–90% of Takt to absorb micro-stoppages without building excess WIP.

Motorola & ISO 13053

DPMO = (Defects ÷ Opportunities) × 1,000,000

Six Sigma assumes an empirical 1.5σ long-term mean shift. Reaching 6.0σ represents 3.4 defects per million opportunities, dramatically reducing reliance on heavy manual sorting inspection.

Defect Compounding Law

RTY = Y₁ × Y₂ × ... × Yₙ

Compounds first-pass yields across sequential processes. Five operations at 95% yield result in 22.6% total defect leakage, proving that single-station metrics blind plant leaders.

Theory of Constraints (TOC)

Line Balance = Total Work ÷ (n × Bottleneck)

Maximum line throughput is mathematically constrained by the bottleneck workstation. Discrepancies between other stations and the bottleneck generate pure balance delay idle waste.
